Jacqueline Silva, PhD Candidate, The University of Texas Medical Branch, Galveston, TX, explains the significance of calcineurin inhibitors (CNIs) in reducing the prevalence of dementia. Using electronic health records from a global research network, data was collected on patients who were treated with tacrolimus or cyclosporine. Although the use of either agent was associated with a reduction in the prevalence of dementia compared to an age-matched general population control, tacrolimus was more effective (18% risk reduction). Its superior effect could be attributed to its brain penetrance, as cyclosporin only has peripheral action. The efficacy of cyclosporin despite its non-brain penetrant nature demonstrates the value of peripheral action of CNIs. The drugs are both well established and are widely used for immunosuppression; therefore, approval for use in dementia prevention could be streamlined if proven effective.
